You can request a copy of your medical records by clicking the link below OR by filling out our paper request form and faxing or mailing it to us. According to South Carolina Law Section 44-7-325, we "shall comply with a request for copies of a medical record no later than forty-five days after the request is received". Our goal is to complete requests within 30 days of receipt. Thank you for your patience.
